Medication Forms
You will need to complete a Release Form every time our staff has to administer medication to your child. Prescription medicine MUST be labeled with the child's name, the name of the medicine, the dosage, physician's name, and length of time medicine is to be administered, as well as any special directions. We CANNOT administer medication to your child if any of this information is missing or incorrect. Non-prescription cough medicine also requires a Release Form. Our staff will administer only the recommended dosage noted on the product label.

Accident/Incident Forms
Forms for accidents or other incidents will be completed when something unforeseen happens to your child. You and the Center's Director will both sign the form and a copy will be kept by each.
